Prior authorisation for euthanasia from insurer/insurance provider


If the horse is insured for death (mortality insurance) the insurance policy may state specific 

requirements which must be fulfilled in order to meet the necessary conditions for a claim.


It is your responsibility as the owner/insurance policy holder, or the person acting on the policy 

holder’s behalf, to ensure that the conditions of the policy are met.

 

These may include:

  • obtaining prior authorisation for euthanasia from the insurance provider.
  • seeking a second opinion from another veterinary surgeon.
  • a postmortem examination being carried out


Post-mortem examination

A post-mortem may be a requirement for insurance claims. 

Euthanasia costs


Costs for euthanasia do vary due to many variables including, age, breed, weight, circumstances for euthanasia  especially in emergency and/or out of hours circumstances. 


An estimate of costs for euthanasia including visit, medication and examination  (excluding collection which is paid directly to the third party) would be in the region of £300-£500 (incl VAT)

Once signed, a copy of this form will be sent to you, and the original submission will be retained by the practice.
